Support at home
Access to community-based services.
There are a wide range of community health and wellbeing services providing care and support for people from birth to the end of life. This includes helping people with more complex health and care needs to live independently in their own homes for as long as possible.
Community health care teams include GPs, community nurses, allied healthcare professionals, social care workers, community pharmacists, care co-ordinators and social prescribers. They work in partnership with a wide range of voluntary, charity and community services.Together these services provide support to best meet the individual needs of the person living with and beyond cancer and their family / friends.
